Registration

Policies & Procedures

You may register your child online or come to the office to receive a registration packet. Below are some important guidelines to assist you with the process.  Forms and more information about registration can be found at the Santa Clara Unified School District website.

New Online Pre-Registration

Santa Clara Unified School District now offers online pre-registration for new students.  If your child has never attended a school in Santa Clara Unified School District, you can now pre-register your child online.  Once you complete your pre-registration, which includes filling out all of the information online, please email Veronica Zepeda at vzepeda@scusd.net or call the office at 408-423-4111.  

After completing the online registration, print out the registration forms, complete the documents below and bring all completed forms with you to the office.

 

You will need the following:

  1. Original Birth certificate or passport (to prove age and correct name spelling)
  2. 3 forms showing proof of residence (address verification) - i.e., PG&E bill, Utility bill, or rental agreement with current month's rental receipt. 
  3. An immunization record (shot record) must be presented upon registration. (Do not wait for immunization record to be completed before registering, you will be given some time to complete shot record). Children will not be registered in a class until immunizations are in compliance.

 

The following immunizations are required:

  • TB skin test - PPD/Mantoux test given within 18 months of beginning kindergarten and within 6 months of beginning PK through grades 1-5.
  • 3 Polio - last booster after 4th birthday.
  • 4 DTP - last booster after 4th birthday.
  • 3 Hepatitis B
    • 1st dose must be received before school entry
    • 2nd dose must be received 1-2 months after 1st dose
    • 3rd dose must be received 6 months after 2nd dose
  • 2 MMR (Measles/Mumps/Rubella) - after 1st birthday
  • Varicella (Chicken Pox) - must be received before school entry (or provide a date of Chicken Pox infection)
    • (Do not wait for immunization record to be completed before registering, you will be given some time to complete immunizations)
